A Framework for Achieving Value-based Care

As the Affordable Care Act demanded that providers and health systems deliver quality care at a lower cost, the Institute for Healthcare Improvement developed a framework that was later expanded to what we now call the Quadruple Aim.1-3 Health Equity is achieved when everyone can attain their full potential for health and well-being. This model is now widely accepted by public and private health organizations as a means for optimizing health system performance.2,3

In recent years, improving health equity and addressing social determinants of health have been recognized as crucial components to achieving the goals set forth by the Quadruple Aim.4,5 As such, efforts are currently being made to include additional strategies, resources, and case studies focused on these important topics to help health system stakeholders in their efforts to deliver quality, equitable care.
